          Job Title:                        SMART Assessor Care Co-ordinator

          Salary Band:                  P11-P20 

          Responsible to:              Poole Treatment Gateway Manager

          Hours:                          37.5 hours per week (to include Saturdays and some unsociable hours on a rotational basis)

            MAIN PURPOSE

To provide comprehensive assessment and full care co-ordination to individuals seeking access to drug and alcohol treatment services in Poole and to refer to the most appropriate treatment provider.

 

            MAIN RESPONSIBILITIES

  • To be part of the Team that is the single point of entry to, and exit from, drug and alcohol services in Poole.
  • To ensure the completion of comprehensive assessments, risk assessments, risk management plans, severity of need review and ensure the development of individual care plans with those who express a wish to access treatment/support services and input this information into a database (HALO). Assessments to be carried out at the SMART building or within community settings.
  • As a priority the safeguarding processes/services for children/adults in Poole must be understood and when necessary actions rigorously implemented.
  • To ensure Care Co-ordination for individuals who have been assessed as requiring such services and appropriate referrals to a range of treatment/community support agencies.
  • To undertake all TOPs starts at assessment and identify when reviews and exits are due with treatment providers via the Halo flag system - completion as appropriate
  • Provide access to specialist information and assistance regarding housing, finance, education, employment and training and families’ relations.
  • Provide preparatory sessions with all service users assigned to structured modalities.  These preparatory sessions will include a review of assessment data, overview of the Poole treatment system and be used to motivate the client to undertake treatment.
  • Provide effective links with mental health, Children’s Services, prescribing, prison, criminal justice, young persons and other voluntary and statutory sector providers and agencies.
  • To support the weekly group programme as and when required.
  • To deliver naloxone training and distribution where appropriate.
  • To support the provision of pre-and post-blood borne virus test discussion and undertake dried blood spot testing.
  • Provide evidence-based harm reduction services, including needle exchange – and complete associated online training
  • Provide Care Co-ordination and monitor the Service users journey both within the criminal justice system and treatment system and proactively work to re-engage Service users, if a service user has disengaged.
  • Provide retail support in the recovery café as and when required.
  • To work in partnership with other agencies to provide a package of support around holistic needs, by attending multi disciplinary team case review meetings when necessary.
  • To actively promote the service of SMART to the community of Poole.
  • To proactively remind service users about appointments, using the HALO text alert system and actively seeking to engage service users who do not attend.
  • To encourage service users to actively participate in service user groups, provide feedback on services and individual sessions and ensure this information is highlighted to project manager.
  • To help support the mentoring process of the volunteers/SMART personnel.
  • Provide support with digital interventions e.g. Live Chat, 24/7 Helpline & digital apps
  • To undertake such other duties as may be required from time to time commensurate with the level of the post.
  • To maintain personal clinical expertise through supervision and appraisal and attend training and development as required.
  • To comply with all decisions and policies of EDAS and any relevant statutory requirements, including Equal Opportunities legislation, the Health and Safety at Work Act and Data Protection Act.
